U.S. Heading for Record Whooping Cough Epidemic

There have already been 18,000 cases of whooping in the U.S. this year, more than double the number seen at this point last year. Nine children have already died. At this rate, 2012 will become the worst year for whooping cough since 1959, when near 40,000 cases were reported.

Prior to the era of mass vaccination for Pertussis (whooping cough), the U.S. saw nearly 150,000 cases per year, with as many as 8,000 deaths. However, for the past decade a mass hysteria about vaccines (see here, here and here) has prompted thousands of parents to forgo Pertussis and other vaccinations for their children, causing a sharp rise in infections and preventable deaths. Ironically, a large percentage of these parents come from well-educated, affluent backgrounds.

For the first time in fourteen years of teaching I am seeing students marked absent by the attendance office for failing to get their required vaccinations. With California’s new requirement that all teenagers get DTAP boosters against whooping cough (pertussis), schools across the state are seeing declines in daily attendance as unvaccinated students are being barred from attending class.

The law makes sound public health sense. Whooping cough is not an ordinary respiratory infection like the common cold. It is often deadly, particularly for infants. In fact, ten children died of the disease in California last year, in the state’s worst outbreak in sixty years. While the majority of infected children do not die, the disease is still far from trivial. Hospitalizations are common, as are protracted absences from school. Students sometimes miss 2-4 weeks of school while they recover from the disease. Furthermore, if an infected student does attend class, approximately 50-80% of all unvaccinated children will contract the disease (see The California Report, KQED, 9/21/11).

The increase in pertussis cases was due primarily to declining vaccination rates among the state’s children and the fact that the vaccine does not confer life-long immunity, like the polio vaccine does. In order to maintain one’s protection, booster shots are required during the teen years. However, many parents used the “personal belief” exemption to get their kids exempted, which allows parents to avoid vaccinations for their kids for virtually any reason at all, including the misperception that vaccines are dangerous.

One rarely discussed repercussion of California’s new vaccination law has been a worsening of schools’ financial woes. Most schools depend heavily on state funding which is based on Average Daily Attendance (ADA). As a result, schools with high rates of absenteeism lose substantial funding.

Some school districts, like the Natomas Unified District near Sacramento, are responding by ignoring the law and allowing unvaccinated students to attend school, in flagrant disregard for the health and safety of their other students and staff (see California Report, KQED, 9/21/11), so they can keep their ADA numbers high. Some have taken the “precaution” of quarantining the unvaccinated students in their gymnasiums or libraries, which is probably still a violation of education code or other state laws requiring equal access. It certainly deprives these students of the same learning environments as their peers and makes lab science classes impossible.

This tactic to save money could end up backfiring. If unvaccinated students infect their peers, a school could have a large outbreak and end up with far more absences than they would if they simply obeyed the law and kept unvaccinated students home.

Contrary to the paranoid delusional perspectives of some parents, vaccines like the DTAP vaccine are not only safe, but do a much better job protecting children from a serious illness than leaving them unvaccinated. If we compare this year with last, there have only been 10 new pertussis cases in Marin County, one of the worst hit (and least vaccinated) counties in the state last year, with 350 cases.

While it is still not entirely clear why they had such a dramatic turnaround, we do know that Marin health officials launched an intensive public education campaign and offered free vaccine clinics to support the new state requirement that all middle and high school students have up-to-date pertussis vaccinations. It is very likely that the new law, combined with the outreach campaign contributed to the dramatic decline.

According to the Bay Citizen, the Marin outreach campaign was quite extensive and included letters to parents, a middle school poster contest, public service announcements and free vaccinations for those who could not afford them. There were also five back-to-school clinics over the summer that provided vaccinations. And it wasn’t just children. Parents got boosters, too, helping to bring the community closer to achieving herd immunity and further reducing the chances of contracting the disease.

However, the problem seems to be growing in other counties. According to the California Report, there are still 2,000 students in San Francisco Unified, 16,000 in San Diego Unified, 45,000 in Long Beach Unified, and 209,000 in Los Angeles Unified who still need to be vaccinated.

Superstition, Pseudoscience and Threat to Children Grow in California

The number of parents getting their kids vaccinated has been declining, particularly in affluent communities, like Marin, in the San Francisco Bay Area. The Bay Citizen has just published vaccination rates for kindergartners in the Bay Area. However, it is not just Marin with lower than normal vaccination rates. Many affluent communities had vaccination rates well below the state average, like Atherton and Burlingame in San Mateo County.

Children are required by law to have proof of vaccinations before being admitted to schools in California. However, parents can still get their unvaccinated kids into school by using the “personal belief” exemption. 21 states have “personal belief” exemptions, according to VaccineEthics.org.

The “personal belief” exemption rate for private schools was 3.56% compared with 2.16% for public schools. However, Marin County had a “personal belief” exemption rate of 7.06% for its public schools, triple the state average and more than double the average of all other Bay Area counties except Sonoma, which had an exemption rate of 5.98%.

Marin County had the highest per capita income in the state, with a 2009 median household income of $86,658. According to the 2000 census, it was also the highest ranked in the nation. San Mateo, San Francisco, Santa Clara and Contra Costa Counties were the 2^nd-5^th wealthiest counties, while Sonoma County came in at number 12, with a median household income of $53,076, which was slightly above the state norm. In 2009, however, the median household income was $61,985, with 9.5% of residents living below the federal poverty level, significantly lower than the 14.2% statewide average. However, wealth is not the only factor influencing vaccination rates. San Francisco and Santa Clara Counties each had exemption rates below 2%, while San Mateo and Contra Costa Counties were just over 2%, despite the fact that they are among the wealthiest counties in the state. On the other hand, the Sonoma towns of Sebastopol and Occidental (both with median household incomes around $60,000, just above the state median), had whopping “personal belief” exemption rates of 28.53% and 33.33%, respectively, helping to skew the data for this small county with a population barely half the size of San Francisco. Other Sonoma towns also had higher than average exemption rates (e.g., Geyserville, 9.38%; Cloverdale, 5.13%; Guerneville, 5.56%; Petaluma, 6.10%).

The “personal belief” exemption allows parents to opt out of vaccinations for almost any reason as long as it is “against their beliefs,” including the mistaken belief that vaccines are risky or dangerous for their children. This belief has been growing recently, despite overwhelming evidence to the contrary, due to a well-funded and politically well-connected snake oil machine, particularly among many white, middle class, anti-establishment and anti-“Western” medicine types (many of whom live in Marin, Sonoma and other Bay Area communities).

The notion that vaccines cause autism has been thoroughly discredited (see here, here and here). The main proponent of this fraud, Andrew Wakefield, has been barred from practicing medicine and had his paper retracted from The Lancet when it became clear that he fabricated data and collected $674,000 from lawyers suing vaccine manufacturers, a serious conflict of interest he failed to disclose to the journal.

Other pseudoscientific paranoias include the notion that vaccines overwhelm a child’s immune system. However, there is no science to back this claim. Some parents also believe that vaccines increase the risk of diabetes, but this is based on the flawed research of a single discredited scientist. The overwhelming scientific consensus is that there is no connection.

In fact, the chances of a serious vaccine-related side effect are far lower than the chances of the disease itself causing serious side effects or death, which is the main reason why children should get vaccinated. There are some minor side effects associated with some vaccines, like muscle pain or fever, but they are usually mild and relatively uncommon.

For any given vaccine, there is also a small chance that it will not give the patient immunity. For most vaccines, this rate varies from 5-25%. However, this is absolutely NOT a reason to forgo vaccines because the more people who are vaccinated, the greater the herd immunity, which gives the greatest protection of all. For example, an unvaccinated person in a highly vaccinated community has greater protection than a vaccinated person does in a community with a low vaccination rate. This is because once the herd immunity threshold has been reached (generally around an 80% vaccination rate) there is little opportunity for the infection to take hold in the community and hence no way for it to spread.

Many parents who forgo vaccines do so under the belief that the “herd” immunity of their community will protect their children. The problem with this belief is that their community may never attain herd immunity because so many are forgoing their vaccinations. It is also a terribly selfish and callous mentality that essentially expects everyone else to do what they fear and refuse to do. “I won’t let my child be damaged by those terrible vaccine manufacturers, but I’m counting on everyone else to get vaccinated so my precious won’t catch something nasty.

Not surprisingly, seven of the 12 California counties with the highest rates of whooping cough last year (including Marin) also had above average rates of “personal belief” exemptions, according to an article by California Watch. The others were rural Central Valley counties with high rates of migrant laborers who it is presumed have lower vaccination rates due to poverty and immigration status. Numerous studies have found that states with easy to obtain “personal belief” exemptions, like California, had higher vaccination exemption rates and higher rates of pertussis (whooping cough) and measles—see here and here.

California’s whooping cough rate has approached levels not seen since the 1950s. The disease can be deadly for infants (10 died of the disease in 2010), a risk that increases dramatically when there are higher than normal rates of infections among older children. However, even older children suffer, with some requiring hospitalization, while extended absences from school can jeopardize their academic success. I had one student who missed two weeks due to whooping cough. He never did catch up and had to retake a semester of biology.

One might wonder why there is even such a thing as a “Personal Belief” exemption, as it implies that one person’s beliefs trump public safety, even when their beliefs are superstitious, selfish or wrong. We do not allow a “personal belief” exemption for bullying—kids cannot get permission to bully others based on race, gender, sexual orientation, even if that’s the belief of their families. Yet many schools do allow students to be excused from studying evolution because of their religious beliefs and exemptions from sex education are relatively common.

All of these exemptions have potentially serious consequences for society. Unvaccinated children decrease the chances of herd mentality being achieved, thus increasing the risks for everyone. Children who are ignorant about sexuality, birth control and STDS are at risk of unwanted pregnancies or STDS and spreading them to other people’s children.

Until recently, however, it was relatively difficult to get an exemption from vaccinations. Parents had to show that they held religious beliefs that prevented vaccinating their children. And there is a movement to tighten these rules in order to increase vaccination rates. The National Association of County and City Health Officials, for example, supports the elimination of “personal belief” exemptions, while leaving religious and medical exemptions intact.

California’s Whooping Cough Epidemic (And New School Vaccination Requirement)

There have already been 733 reported cases of whooping cough (Pertussis) in California this year, with 78 cases just in Contra Costa County, according to the Bay Citizen. In 2010, the county had 205 cases for the entire year, while there were over 8,000 cases throughout California, the most since 1947. Last year, 10 infants died of the disease in California.

Pertussis is a serious respiratory infection that is treatable with antibiotics and preventable with vaccines. While it can be deadly for babies, it can still cause healthy older children and adults to miss weeks of school or work and even require hospitalization. There has been an upsurge of infections in California among some unvaccinated immigrants and especially among unvaccinated middle class native born children whose parents ostensibly have bought into the anti-vaccine hysteria.

A new state law now requires students in grades 7 through 12 to show proof of vaccination for Pertussis before they go to school, including a recent booster. (The dtap vaccine given to toddlers does not give life-long protection against whooping cough, as do the vaccines for mumps and measles). State Superintendent Tom Torlakson recently produced a video message to inform parents of the new law and the dangers of Pertussis.

California Now Requires Whooping Cough Vaccines

More crazy news from crazy California: We're now going to require kids to be vaccinated against whooping cough before allowing them to come to school and infect their peers and teachers. That this wasn't already law is probably news to many. The question is: Will the middle class mommies who erroneously believe that vaccines give their babies autism and other horrible diseases choose to quit work and home school their kids?

California teens need to be up to date on their whooping cough shots before starting classes next fall in order to be in compliance with a new law.

The statute requires all middle and high school students to be vaccinated against the highly infectious disease, also known as pertussis, prior to the school year that starts in the fall of 2011.
State public health medical officer Dr. Eileen Yamada says students in public and private schools will need to show proof of a TDAP booster shot before starting school.

"They really will need documentation to get into school, so it's very important to plan ahead, make appointments now, and keep student immunization record in a safe place," said Yamada.
Parents should scrutinize medical records to make sure their children got the TDAP vaccine, which covers pertussis, and not just the TD vaccine.

Murderous Middle class Marin Mommies

Marin county ranks #2 in California for pertussis (whooping cough), with 123 cases per 100,000. The illness has reached epidemic levels in California, with 5600 cases and 10 deaths. The disease is completely preventable with the dtap vaccine. However, bourgeois parents in Marin often choose to forego the vaccine out of irrational fears about the safety of vaccines (and the assumption that enough other parents will vaccinate their children to protect their unvaccinated kids through herd immunity). Many are also skipping the vaccine for measles, a disease that once killed 3000 children annually.

In San Francisco, and many other counties, the majority of those stricken with the illness are Spanish speaking immigrants. The assumption of medical professionals is that there is an outreach problem in terms of educating the immigrant population. There has also been a dramatic jump in adult infections due to the fact that the vaccine wears off over time and adults rarely get boosters. The high infection rate in Marin, however, is primarily among white middle class children.

What’s most troubling about the anti-vaccine hysteria is the utter disregard for everyone else’s safety. It is understandable that parents would want to protect their children from perceived threats. But in the case of vaccines, not only is the perceived threat irrational and exaggerated, but the consequences of foregoing vaccines are that everyone’s risk level goes up proportionately to the number of people who are not vaccinated. 

Herd immunity occurs when 80-90% of the population is vaccinated (depending on the pathogen). Once a threshold of vaccination has been reached, it is virtually impossible for the pathogen to establish a foothold in the population, thus protecting the minority that has not been vaccinated. Some studies have indicated that being unvaccinated in a highly vaccinated population is far better protection than being vaccinated in a poorly vaccinated population. Therefore, a selfish and self-entitled parent might assume that his or her child is safe, even if the child is unvaccinated. The problem is that if too many parents make this assumption, the threshold is not met and herd immunity does NOT occur and their child is at an increased risk of contracting a potentially deadly disease. Worse, everyone’s children are at an increased risk.

The anti-vaccine movement is dominated by wing nuts and extremists who use fear and misinformation to intimidate opponents and to recruit new adherents. Amy Wallace, author of Epidemic of Fear, and Paul Offit, coinventor of the rotovirus vaccine (a drug that could save thousands of children’s lives), were recently sued for $1 million dollars by Barbara Loe Arthur of the National Vaccine Information Center (NVIC) for liable. Fortunately, the lawsuit was thrown out. Robert F. Kennedy Jr. called Offit a “biostitute,” while actor, Jim Carey, calls him a profiteer. One man even threatened to murder him under the mistaken assumption that this would somehow save innocent children’s lives.

Why so much sturm und drang over an award winning scientist who has created a life-saving medication? Autism is a terrible disease. It scares the hell out of new parents who are understandably desperate to find a causative factor they can blame. Offit has infuriated anti-vaccination activists by publishing well-researched data refuting their bogus claims, including the one linking autism to vaccines. Numerous studies have shown that there is no link between vaccines and autism. The illness is typically diagnosed in children between the ages of 12 and 24 months, the same ages in which they are receiving common childhood vaccinations. The correlation however, is purely coincidental. Thimerisal, a mercury-containing preservative, has not been used in children’s vaccines for over ten years, yet autism rates have continued to climb in that same time period. Likewise, vaccines have been incorrectly linked to Guillain-Barre syndrome, after several people contracted this tragic auto-immune condition coincidentally after receiving swine flu vaccines in the 1970s. (Guillain-Barre is extremely rare, occurring in only one to two out of every 500,000 people. The cases in the 1970s did occur after flu vaccinations, but were still within the normal range).

Anti-vaccination advocates also blame Big-Pharma for sacrificing their children’s safety for profits. This fear is based on a long history of pharmaceutical companies distorting data, providing false claims of safety, and massively promoting dangerous products in order to boost profits (e.g., Vioxx, Fen-Phen, Accutane, DES, Thalidomide). While drug companies have been making fortunes from drugs like Lipitor, Nexium, Prozac, and Viagra, most have been getting out of the vaccine market, as profits are low compared to the blockbusters, and liability is high, particularly for newer vaccines, like influenza, which must be reformulated each year. If anything, the profit motive has increased our risk of contracting preventable diseases by limiting availability (See Mike Davis, Monster at the Door, for how this relates to influenza).

Many parents believe that too many vaccines overwhelm a child’s immune system. There is no science to back up this claim. However, a newborn may not have a strong enough immune system to develop immunity when vaccinated, so the hepatitis vaccine that hospitals offer the day after a child is born may not be effective. Also, parents can choose to space out the vaccinations to allow their children to recover in between vaccinations (many do cause minor side effects like muscle pain or cold symptoms). Some parents also believe that vaccines increase the risk of diabetes, but this is based on the flawed research of a single discredited scientist. The overwhelming scientific consensus is that there is no connection.

Vaccines save lives. Consider the smallpox vaccine, which has eradicated this deadly disease throughout the world, or the polio vaccine, which has eliminated this potentially deadly or disabling disease throughout most of the world. Avoiding vaccines for your child not only increases your child’s risk of contracting a deadly disease, but increases the risk for everyone else, too. There are also other costs associated with non-vaccination. Diseases like whooping cough, for example, which tend to kill mostly infants, can cause older children to miss weeks of school, potentially causing them to fail and have to repeat a year of classes. Parents may be forced to skip weeks of work to care for sick children (or themselves, if they catch the disease). Buying into pseudoscientific quackery and snake oils will not prevent or cure autism.
